Applicants are requested to file their application for the examination before the desired schedule at the UB College of Nursing office.
Please prepare the following:
1. Medical Certification
2. Entrance Examination Application Form (CON Office)
3. 2 x 2 picture with name tag
4. PSA Birth Certificate
5. Examination Fee of Php250.00
6. Long Brown Envelope
For TRANSFEREES, all other requirements stated above with the additional requirements:
1. TOR
2. Honorable Dismissal
