• Form 138 (Senior High School Card)
• Certification of Good Moral Character from the Senior High School Principal
• Original & Photocopy of Birth Certificate/PSA
• 2 pcs. Black & White ID (2″x2″) with white background

Transferees:

• Honorable Dismissal or Transfer Credentials
• Transcript of Records for Evaluation
• Original & Photocopy of Birth Certificate/PSA
• Certification of Good Moral Character from Dean of previous institution
• 2 pcs. Black & White ID (2″x2″) with white background
