Admission Requirements - UB Grade School


  1. Age
    1. Nursery – Age must be 3 years old and below
    2. Kindergarten 1 – Age must be 4 years old by August 2020
    3. Kindergarten 2 – Age must be 5 years old by August 2020
    4. Grade 1 – Age must be 6 years old and above and must have taken
      Kindergarten from a DepEd accredited school

  2. Credentials
    1. Resident/Old Pupils
      • Duly Accomplished Enrollment Form
    2. New Pupils/ Transferees
      • Duly Accomplished Enrollment Form
      • Report Card (Form 138)
      • Original Copy of NSO/PSA issued birth certificate
      • Recent 2x2 colored ID picture
      • Certificate of good moral character from the school last attended
