- Nursery – Age must be 3 years old and below
- Kindergarten 1 – Age must be 4 years old by August 2020
- Kindergarten 2 – Age must be 5 years old by August 2020
- Grade 1 – Age must be 6 years old and above and must have taken
Kindergarten from a DepEd accredited school
- Credentials
- Resident/Old Pupils
- Duly Accomplished Enrollment Form
- New Pupils/ Transferees
- Duly Accomplished Enrollment Form
- Report Card (Form 138)
- Original Copy of NSO/PSA issued birth certificate
- Recent 2x2 colored ID picture
- Certificate of good moral character from the school last attended
