      • 2 pcs 2x2 ID picture

    2. Elementary / Junior High School / Senior High School
      • Duly Accomplished Enrollment Form
      • Certificate of Good Moral Character
      • Birth Certificate issued by Philippine Statistics Authority (PSA)
      • Report Card (no failing grades in all subjects in any grading period)
      • 2 pcs 2x2 ID picture
      • Permit to study for Foreign Students
      • ESC certificate for high school (if ESC grantee)
